Follow these 4 simple steps below.
Obtain the required forms in order to submit a policy servicing request.
Make sure all fields are completed and to attach all relevant supporting documents.
Submit completed forms with supporting documents and all necessary payments (if any). Find payment options in the FAQ section.
Contact your Financial or Life Insurance consultant to check on your policy status.

You need to make sure all the fields are completed and attach relevant supporting documents such as the proof of identity and latest medical report. You can submit the completed form and supporting documents to us through your Sale staff at ACLEDA Bank Branch, Individual Insurance Agent or at our Prudential Cambodia Customer Service Centre.
You can apply changing your information or reinstatement of your policy by simply downloading the application form in the above section, take a selfie with your ID card, take a close up photo of your ID card, and send them to our email via pcla.alteration@prudential.com.kh.
