  3. Physician’s statement of TPD, using company’s form
  4. Copy of PO’s bank account
  5. Copy of PO’s ID card
  6. Copy of police report of insured event

Claim team is responsible to assess the claim request within thirty (30) days starting from the Claim Registration Date subject to its receiving all the necessary forms and documentary evidence.

Disability due to disease

  1. Claim Request, using company’s form
  2. Claim Event Notice, using company’s form
  3. Authorization Letter (Both Khmer and English), using company’s form
  4. Physician’s statement of TPD, using company’s form
  5. Copy of PO’s bank account
  6. Copy of PO’s ID card
